We are seeking an experienced Registered Nurse to join our home health team in a dual role as RN Case Manager and Quality Assurance Nurse. The ideal candidate will have strong home health experience, excellent clinical documentation skills, and proficiency with Axxess software. This position is responsible for patient case management, clinical oversight, chart auditing, and ensuring compliance with agency, state, and federal regulations.
Position Responsibilities:
Case Management Duties:
- Perform patient admissions, reassessments, supervisory visits, recertifications, and discharges.
- Develop, review, and update individualized care plans.
- Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members.
- Monitor patient progress and communicate changes in condition.
- Ensure timely completion of clinical documentation.
- Provide patient and caregiver education.
Quality Assurance Duties:
- Review clinical documentation for accuracy, completeness, and compliance.
- Audit OASIS documentation and ensure consistency.
- Monitor physician orders, care plans, visit frequencies, and supervisory visit compliance.
- Identify documentation deficiencies and follow up with clinicians.
- Assist with chart preparation for audits and surveys.
- Support compliance with Medicare, ACHC/JCAHO, and state regulations.
- Participate in performance improvement and quality assurance activities.
Qualifications:
- Active Florida Registered Nurse (RN) license.
- Minimum 2–3 years of home health experience required.
- Strong knowledge of home health regulations and compliance.
- Experience with Axxess required.
- OASIS experience strongly preferred.
- Excellent organizational and time management skills.
- Strong attention to detail and chart review experience.
- Ability to work independently and manage deadlines.
Preferred Qualifications:
- Previous QA or Clinical Manager experience.
- Experience preparing charts for audits or accreditation surveys.
- Knowledge of Medicare Conditions of Participation.
